Confidentiality and data protection statement:
I will not reveal information about what is talked about in your therapy, except where life or safety are immediately threatened. In such circumstances I will make every effort to talk to you about this first.
As a mandatory requirement of the professional bodies of which I am a member, I receive regular professional supervision of my work. Any material discussed with my supervisor is in an anonymous form and is not traceable to you or your records.
Your personal contact details are kept in a password protected file. In addition to this, session records made by me, are stored separately using a code so they cannot be linked to your name and kept in a password protected environment.
I store, using confidential accounting software via my bank, your name and billing address for the purposes of producing invoices for you. Your name will also appear on bank statements submitted for tax purposes to HMRC, if you wish to use a reference instead of your name, please request this.
I entrust the contact details of all my clients to a professional colleague so that, in the event of my death or incapacitation, you will be informed, and if appropriate, offered information about finding alternative professional help. This colleague will also oversee the destruction of any records pertaining to you in a confidential manner.
Complaint Policy regarding processing your data:
If you wish to make a complaint about how I am processing your personal date you can raise this with me in person or via email: david.petherbridge@outlook.com
I will:
• acknowledge receipt of complaints within 30 days of receiving them;
• without undue delay, take appropriate steps to respond to complaints, including making appropriate enquiries, and keep people informed; and
• without undue delay, tell people the outcome of their complaints.
